2. Required Documents

Before registering, New York State Education law requires the following documents be presented:

  • Original documentation of birth, in the form of an original birth certificate, passport or baptismal record
  • Official immunization record signed by a physician or clinic staff
  • Custody papers (if applicable)
  • Proof of residence which includes the name and address of the parent(s) or guardian(s) and is dated within 30 days (recent date). Examples of proof of address include the following:
    • Verizon Phone Bill – correct address and name of parent(s)/guardian(s) must be on the bill, and must contain recent date
    • Auto Insurance ID Card – Insurance ID card but be valid and with the name and address of the parent(s)/guardian(s)
    • Lease Agreement – Legal and valid lease agreement between owner and recent date
    • National Grid Bill – Bill must contain address and name of parent(s)/guardian(s), must contain recent date
    • Closing Papers – Documentation of final closing with name of parent(s)/guardian(s) and address, must contain recent date
    • New Drivers License
    • Cable Installation
    • Notarized Proof of Temporary Residency

Items NOT Accepted:

  • Bank accounts
  • Rent receipts
  • Mail
  • Tax bill
  • Payroll checks
